Blood test indicates stroke susceptibility

MedWire News: A blood test for two proteins that are associated with inflammation could help identify people who are at risk of suffering a stroke, researchers report.
Dr Vijay Nambi (Baylor College of Medicine, Houston, Texas) and colleagues found that testing for the two proteins ? called high-sensitivity C-reactive protein (hs-CRP) and lipoprotein-associated phospholipase A2 (Lp-PLA2) ? improved the accuracy with which they could predict whether people would suffer an ischemic (clot-related) stroke within five years.
The team assessed 949 men and women who were taking part in the Atherosclerosis in the Communities (ARIC) study. A range of factors traditionally used to determine stroke risk were evaluated, including smoking, blood pressure, cholesterol levels and diabetes. Blood samples were also tested for hs-CRP and Lp-PLA2. Five years later, 183 (19%) of the participants had suffered an ischemic stroke.
First, the team examined whether the addition of information about hs-CRP and Lp-PLA2 levels changed the way in which stroke risk was classified. Using only the traditional risk factors, 86% of participants were classified as at low risk of suffering a stroke within 5 years, 11% had an intermediate risk and 3% had a high risk. However, after taking blood test results into account, approximately 14% of the participants were reclassified into a different risk category. This related mainly to movement into or out of the ?intermediate risk? category.
Next, Nambi and colleagues looked at the accuracy of this reclassification, based on the number of people who actually experienced a stroke within 5 years. They found that classification of stroke risk including information on hs-CRP and Lp-PLA2 was significantly more accurate that that based on traditional risk factors alone.
?While more studies must be done to determine if these categories can be used as a standard, these results show us one strategy to classify an individual's stroke risk and how this can be altered by the addition of novel blood tests,? Nambi concluded.
